Dr. Cook likes to see new patients for an initial 60 minute consultation to assess what your needs are and if they can be met in his private outpatient office. If Dr. Cook recommends any medications or medication changes, he generally likes to see you back in the office in one to three weeks for a 25 minute follow up session. Follow ups are generally scheduled once every three months as long as you are doing well. The initial consultation is $450.00 and we do accept Cash, Check, Visa, MasterCard or American Express. 25 minute follow ups are $200.00, and 45 minute sessions are $300.00 If you do have private insurance, we will file it as a courtesy to you and any reimbursements will be sent directly to you. Dr. Cook is not a network provider and does not accept Medicare or Medicaid. We have a 24-hour cancellation policy; any changes or cancellations must be made a full 24-hours in advance of the appointment. We require you to secure the appointment with a credit card. We will not charge the visit to the card unless you do not show or cancel with less than 24-hours notice. This is for securing your appointment only.

The initial consultation is $250.00 and we do accept Cash, Check, Visa, MasterCard or American Express. Follow ups are $145.00. Follow ups are generally scheduled weekly at first. If you do have private insurance we will file it as a courtesy to you, and any reimbursements will be sent directly to you. We do have a 24 hour cancellation policy. If you are unable to make your appointment, please call the office outside of a 24 hour period to cancel your appointment. Nancy asks that all patients secure the initial appointment with a credit card. The card will not be charged unless the appointment is missed or cancelled without 24 hour advanced notice.
